Proper airway positioning is one of the simplest and most effective methods of improving airway patency in trauma patients. Before advanced airway interventions are considered, providers should ensure the airway is properly positioned and reassess for improvement.

Positioning maneuvers help relieve airway obstruction caused by the tongue, soft tissues, blood, vomit, or other factors that may interfere with normal airflow.

Common airway positioning techniques include:

  • Head Tilt-Chin Lift
  • Jaw Thrust Maneuver

The Head Tilt-Chin Lift maneuver is commonly used to open the airway in patients without suspected cervical spine injury. By extending the head and lifting the chin, providers can move the tongue away from the posterior pharynx and improve airflow.

The Jaw Thrust Maneuver is preferred when cervical spine injury is suspected. This technique opens the airway by lifting the mandible forward while minimizing movement of the head and neck.

Additional airway positioning considerations include:

  • Maintain cervical spine precautions when indicated.
  • Reassess airway patency after positioning.
  • Suction the airway if blood, vomit, or secretions are present.
  • Utilize airway adjuncts when positioning alone is insufficient.

Patients with significant facial trauma may require additional airway interventions due to swelling, bleeding, disrupted anatomy, or airway obstruction. Frequent reassessment is essential because airway compromise can worsen over time.

Patients who are vomiting or have excessive secretions may benefit from positioning that allows drainage while maintaining airway patency. Providers should balance airway management needs with spinal motion restriction considerations and patient condition.

The recovery position may be considered for patients who are unconscious, breathing adequately, and able to maintain their airway when no immediate airway intervention is required. Proper positioning can help reduce aspiration risk and facilitate drainage of blood, vomit, and secretions.
