Modern trauma care continues to evolve, with increased emphasis on early hemorrhage control, balanced resuscitation, and rapid transport to definitive care. In addition to controlling bleeding and supporting airway and breathing, providers should be familiar with tools that may improve outcomes in patients experiencing traumatic hemorrhage and shock. 

Common trauma resuscitation tools include:

  • Tranexamic Acid (TXA)
  • Blood products
  • Fluid resuscitation

Tranexamic Acid (TXA) is an antifibrinolytic medication that helps stabilize existing clots by reducing clot breakdown. TXA does not create new clots — it helps preserve the body’s natural clotting process in patients experiencing significant hemorrhage. 

TXA may be considered in patients experiencing or at risk for severe bleeding. The greatest benefit occurs when TXA is administered as early as possible following injury. Follow current local protocols regarding patient selection and administration. 

Blood products are increasingly recognized as the preferred resuscitation fluid for significant hemorrhagic shock. Whole blood and blood component therapy restore oxygen-carrying capacity, replace clotting factors, and support perfusion. 

Fluid resuscitation remains a component of trauma care. Excessive crystalloid administration may be associated with adverse outcomes — follow current trauma resuscitation guidelines and local protocols. 

The goal of trauma resuscitation is to maintain perfusion, support oxygen delivery, and prevent further deterioration while rapidly transporting the patient to definitive care.

Remember: Stop the bleeding, support perfusion, and get the patient to definitive care.
